Dance style, name of performance and name of performer (first & last name and stage name) must be submitted for program, adjudicator, and award purposes.

Registration fee includes:

• Mini Miss Diva Contestant Sash

• Group rehearsals: Training for model walk and Dance Choreography

• Photoshoot and videoshoot with make up artist
